Tirzepatide Explored in Rare Post Bypass Hypoglycemia Cases

A newly published case report describes three patients who developed postbariatric hypoglycemia after Roux en Y gastric bypass surgery and were subsequently treated with tirzepatide, according to the case report in JCEM Case Reports.

Postbariatric hypoglycemia is a recognized complication that can appear months or years after gastric bypass, when altered gut anatomy leads to exaggerated insulin release and dangerous drops in blood sugar after meals. Management has traditionally relied on dietary changes, acarbose, or other glucose lowering strategies, with variable success. Tirzepatide, a dual GIP and GLP 1 receptor agonist approved for type 2 diabetes and weight management, works through a different mechanism than these older approaches, which is part of why researchers are interested in whether it could help stabilize glucose swings in this population.

It is worth being clear about what this report actually shows. Three patients is an extremely small sample, and case reports like this one are meant to generate hypotheses rather than prove that a treatment works broadly.
