FDA Flags Safety Concerns With Unapproved GLP1 Weight Loss Drugs

The FDA has issued a consumer alert outlining safety concerns tied to unapproved and compounded versions of GLP1 receptor agonists sold for weight loss, according to an FDA statement. The agency notes that compounded drugs are never reviewed for safety, effectiveness, or quality before reaching patients, and should only be used when an FDA approved option cannot meet a patient's medical needs.

Two compounds get specific attention. Retatrutide and cagrilintide cannot legally be used in compounding under federal law, the FDA states, since neither is a component of an approved drug and neither has been found safe and effective for any condition. The agency says it has warned telehealth companies for marketing these compounds directly to consumers, along with ingredient distributors and outsourcing facilities involved in their supply chain.

The alert also flags practical risks: multidose vials used beyond 28 days after first use, injectable products arriving warm due to inadequate refrigeration during shipping, and fraudulent labels listing pharmacies that do not exist. FDA has created an import alert targeting foreign made GLP1 active ingredients with quality concerns.

None of this concerns an approved therapy decision. It is a warning about products operating outside the FDA's review process entirely.
